Allen Cheng, DDS, MD
President
Dr. Cheng earned his medical and dental degrees at the University of California, San Francisco. After completing his residency in Oral and Maxillofacial Surgery, Dr. Cheng finished fellowship training in both Head and Neck Oncology and Microvascular Reconstructive Surgery. He is a Board Certified Diplomate of the American Board of Oral and Maxillofacial Surgery. Dr. Cheng’s practice is comprised of Oral Surgery (dental extractions, dental implants, bone grafting, soft tissue grafting), Head and Neck Oncology, Head and Neck Reconstruction, Maxillofacial Trauma, and Jaw Alignment (Orthognathic) Surgery. His research interests are focused in the areas of Head and Neck Oncology and Reconstructive Surgery. Dr. Cheng currently serves as the Section Chair of Oral and Maxillofacial Surgery at Legacy Emanuel Hospital and Medical Director of the Legacy Good Samaritan Oral/Head and Neck Cancer Program. He is also an Assistant Professor at the OHSU School of Dentistry in the Department of Oral and Maxillofacial Surgery. When he’s not happily helping patients, Dr. Cheng enjoys spending time with his wife and their two children. When he and his wife are not working, they are constantly looking for child-friendly activities to consume the limitless energy exhibited by their two children.
Phillip Mann, DDS, MD
Vice President
Dr. Mann is a Board Certified Oral and Maxillofacial Surgeon, having completed a six-year residency program at Oregon Health and Sciences University, where he also received the MD degree. Prior to that he attended dental school at Columbia University College of Dental Medicine in New York City. Dr. Mann has been in practice in the Portland area since 2015, both in private practice and as an Assistant Professor at OHSU. He has also volunteered with Free to Smile, a non-profit organization providing cleft lip and palate surgery, for a mission to Guatemala in 2014. A native of New Jersey, Dr. Mann and his wife Dr. Tricia Ray enjoy their newfound home in Oregon, along with their two young children Evelyn and David. When out of the office, Dr. Mann enjoys road and mountain biking, running, hiking, and camping throughout the Pacific Northwest.
Gabriel M. Kennedy, DMD

Dr. Gabriel Kennedy was raised in eastern Oregon and attended Oregon State University for his undergraduate studies. He graduated from the University of Connecticut School of Dental Medicine.
He completed his Oral and Maxillofacial Surgery training at Gundersen Lutheran Medical Center in La Crosse, Wisconsin.
Dr. Kennedy is a Board Certified Oral & Maxillofacial Surgeon
Dr. Kennedy has been a contributing author for an Oral and Maxillofacial Surgery manual and has given lectures to the dental community. He is a member of the American Dental Association, American Association of Oral and Maxillofacial Surgeons, and Western Society of Oral and Maxillofacial Surgeons. He is on staff at several local hospitals. He has served on multiple oral surgery missions in developing countries.
Dr. Kennedy met his wife Nicole while growing up in Pendleton, Oregon. They have one son and three daughters. His interests include hunting, snowmobiling, and family activities.
Thomas A. Kolodge, DDS, MD

Dr. Kolodge was raised in Beaverton, Oregon and graduated from Aloha High School. He attended George Fox University in Newberg on a soccer scholarship and earned his degree in Biology. He lived in New York City, while earning his dental degree, attending New York University School of Dentistry. Dr. Kolodge went on to complete medical school and residency in Oral & Maxillofacial Surgery at the University of Nebraska Medical Center in Omaha, Nebraska. He immediately returned home to Oregon and has been in practice since 2009 in the cities of Newberg and McMinnville, Oregon. In his spare time, Dr. Kolodge enjoys adventuring with his wife, Sarah, and their two sons. Anywhere from the mountains to the beach, they love to spend their days outside. When not exploring, he loves to rock climb, golf, and sing for his beloved Portland Timbers with the rest of the Timbers Army.
Daniel Petrisor, DMD, MD

Dr. Petrisor is a board-certified surgeon with training in microvascular reconstructive surgery, head and neck surgery and oral and maxillofacial surgery. He holds a dental degree from Oregon Health & Science University and a medical degree from the University of Texas Southwestern Medical School. In addition to medical school, he also completed his residency at the University of Texas Southwestern Medical Center and Parkland Memorial Hospital in Dallas, TX. Following residency, Dr. Petrisor completed a fellowship in head and neck surgery and microvascular reconstructive surgery at Louisiana State University Health Sciences Center in Shreveport. He then completed an additional fellowship in microvascular reconstructive surgery at the University of Florida-Jacksonville.
Dr. Petrisor has a special interest in treating patients with benign and malignant tumors of the head and neck. He also has training in reconstruction of complex deformities of both hard and soft tissues. This combination of advanced training allows for a patient’s own tissues to be used for reconstructive surgery at the same time as their cancer surgery. Additionally, he is part of a team of physicians, dentists, nurses, nutritionists, therapists and social workers all working together to provide each patient the latest in comprehensive treatments for a variety of cancers of the mouth, head and neck.
Russell Lieblick, DMD

Dr. Lieblick, a native Oregonian, completed his undergraduate degree at the University of Oregon in 1995. He graduated from the University of Louisville School of Dentistry in 2000 and completed his surgical residency at MetroHealth Medical Center in Cleveland, Ohio.
Licensed in Oregon and Washington, Dr. Lieblick is on the medical staff of Mount Hood Medical Center as well as the downtown Legacy Hospitals, Providence Hood River Memorial Hospital, Providence Milwaukie Hospital, and Mid-Columbia Medical Center. Dr. Lieblick is board certified by the American Board of Oral and Maxillofacial Surgeons and is president of Oregon Society of Oral and Maxillofacial Surgeons. He is a Member of the American Association of Oral and Maxillofacial Surgeons, the Western Society of Oral and Maxillofacial Surgeons, the Oregon Dental Association, the American Dental Association and the Multnomah Dental Society. He is a Fellow of the American Society of Dental Anesthesiologists.
When away from the office, he is busy playing with his two children and enjoying activities with his wife, Nina. They enjoy visiting friends and family from all over the country and like to expose their children to as many new activities as possible.
Michael Doherty, DDS, FACS

Dr. Doherty earned his Bachelor’s degree from Gonzaga University and his dental degree from the University of North Carolina. Prior to his fourth year of dental school, Dr. Doherty completed an externship with the OMS department at the Oregon Health Sciences University. After dental school, he completed a General Practice Residency at the National Naval Medical Center in Bethesda, MD. Dr. Doherty gained experience as a general dentist while serving in the U.S. Navy prior to entering OMS residency.
Dr. Doherty completed his Oral & Maxillofacial Surgery training at the National Capital Consortium (National Naval Medical Center/Walter Reed Army Medical Center) in Washington, DC. During training, Dr. Doherty was Chief OMS Resident at the D.C. military hospitals which provided definitive care to many injured sailors, soldiers, and marines. Injuries of such severity are rarely seen in civilian hospitals. Dr. Doherty continually applies knowledge gained from this experience into all aspects of his surgical practice and feels honored to have provided care to such deserving individuals.
He performs surgery in the state-of-the art office which was built in 2016, at Asante RRMC, and at the Surgery Center of Southern Oregon.
Dr. Doherty has authored numerous textbook chapters and journal manuscripts and is a recognized lecturer in maxillofacial trauma reconstruction and dental implantology.
David W. Howerton, DMD

Dr. David Howerton was raised in Portland, Oregon and attended Portland State University for his undergraduate studies. He graduated from Oregon Health & Sciences University School of Dentistry.
He then completed a one year general practice residency at Loma Linda Medical Center. After this he returned to Oregon and completed his Oral & Maxillofacial training at Oregon Health & Sciences University.
Dr. Howerton is a Board Certified Oral & Maxillofacial Surgeon
Dr. Howerton has published numerous scientific articles and is active in lecturing for the dental community. He became a diplomate of the American Board of Oral & Maxillofacial Surgery in 1991.
He is active in the Oregon Dental Association. He is a member of the American Dental Association, American Association of Oral & Maxillofacial Surgeons, Western Society of Oral and Maxillofacial Surgeons, and the Oregon Society of Oral & Maxillofacial Surgeons. He is on staff at several local hospitals and clinics. He is a clinical assistant professor in the Oral & Maxillofacial Surgery Department at Oregon Health & Sciences University in Portland, Oregon.
Dr. Howerton is the proud father of three boys including twins. His interests include basketball, traveling, snow skiing, fly fishing, kayaking, biking, and family activities.
Normund K. Auzins, DDS, FICD

Dr. Auzins graduated from Indiana University Dental School in 1996 and completed his Oral & Maxillofacial Surgery Residency in 2000. He became Board Certified in 2002, and renewed his certificate in 2012. He served the United States Navy until 2003 with two tours to the Middle East. He was stationed on the aircraft carrier, USS Theodore Roosevelt and had a faculty position at Portsmouth Naval Hospital.
He has been in private practice since 2003 and teaches at OHSU part–time. At the state level, he currently sits on the Multnomah Dental Society, the Free to Smile Foundation, and the Oregon Society of Oral and Maxillofacial Surgery. He is also a member of the anesthesia committee of the Oregon Board of Dentistry.
Dr. Auzins patient care extends beyond his practice in Portland to those around the world. Through the Free to Smile Foundation, he is helping patients in developing countries by providing cleft lip and palate surgery. As part of an all volunteer team, he has treated children in China, Tibet, Guatemala, and Niger. Locally Dr. Auzins volunteers for Medical Teams International, providing oral surgery in their dental van for people in need. He and his team routinely participate in the Mission of Mercy (MOM) 2010, 2011, 2013, 2016, Dr. Auzins and his family enjoy spending time in the great outdoors. He enjoys downhill skiing, cycling, hiking, and is also an amateur jazz drummer.
